Job description

Coordinate appointment schedules with surgeons for pre-vascular access evaluation assessment and follow up after vascular access placement. Provide advanced and expert vascular access care to patients Review patients current vascular access status and formulate in conjunction with patient and multidisciplinary team short and long term vascular access plans. Participate in early planning and scheduling of vascular access creation, namely Arteriovenous Fistula or Arteriovenous graft. Monitor pre-operative management of access creation. Assist the Nephrologist in placement and removal of temporary vascular access. Pre-planning for new vascular access intervention before current vascular access fails. Assist surgeons and interventional radiologists with choice of vascular access to be created or monitoring of vascular access post creation. Participate actively in nursing and/or hospital committees and work groups. Demonstrate initiative in the performance of duties. Liaise between the multidisciplinary dialysis vascular access team. Coordinate procedures with Operating Room, interventional radiology, and Bed Management. Coordinate timing of access placement based on laboratory results and dialysis needs. Review census of newly diagnosed End-Stage Renal Disease (ESRD) patients. Attend multidisciplinary dialysis vascular access & hemodialysis meetings. Participate in clinical physician s rounds. Work with hospital discharge planning and case managers to ensure continuity of care is maintained post discharge. Conduct and assess patient/family educational needs regarding safe care of vascular access to maintain longevity, readiness and discharge planning needs utilizing appropriate resources. Advocate for excellence in patient care and for patient s rights. Develop and maintain professional communication with patients, families, and multidisciplinary team. Provide education to all members of the multidisciplinary health care team, patients, and caregivers. Collaborate, guide and advice clinical dialysis/ward staff regarding strategies to prevent and manage vascular access complications. Participate in disseminating vascular access modalities education in the clinic. Maintain up-to-date knowledge of nursing practices, regulatory standards and organizational direction to support users and process teams. Maintain the required clinical practice as a nurse and vascular access coordinator. Gather and review medical records for surgical referral under nephrologist supervision. Develop policy and procedures in collaboration with multidisciplinary health care team based on current evidence based international standards. Maintain vascular access history database. Routinely monitor and act on dialysis treatment data and vascular access adequacy data. Participate in clinical audit as required. Serve as a professional role model, teacher and resource person to nursing staff. Demonstrate respect, compassion and cultural sensitivity to patients/families, other hospital employees, students and faculty. Assume responsibilities for own professional growth, decisions and actions.
