Job description

• Conduct thorough medication reviews to identify potential drug interactions and ensure patient safety. • Advise healthcare professionals on the selection and dosage of medications based on patient-specific factors. • Provide medication counseling to patients, educating them on proper usage, side effects, and storage of their prescriptions. • Monitor patient outcomes post-medication initiation, adjusting therapies as needed to optimize efficacy and safety. • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution is essential for this role. • State licensure as a registered pharmacist is mandatory to practice legally and safely. • A minimum of 2 years of experience in a clinical pharmacy setting is preferred to ensure competency. • Familiarity with electronic health records (EHR) and pharmacy management software is a significant asset.
